Enemy ai prevents following and certain traps don't activate.
The protection spell allows you to walk through enemies unharmed.

I'm not exactly sure what you're asking about the hitboxes on injury.
To put it simply, the hitboxes are all roughly 10x10.
The bats are indeed 1 hit 1 kill. Not all enemies are though. The ones that aren't blink like tim-tim does when hit. (or change as the case may be)
