If you're drawing graphics in an external program, you should use your game's palette, preferably right from the beginning. For best results, use a pixelating program that can do paletted gradients, dithers and transparency, like GrafX2. At the very least, if you draw your graphics in 24-bit colour, convert them to 8-bit using a program with intelligent colour reduction (dithering), don't let Custom do it.
That might be it then. Before importing anything into Custom, I make sure that everything matches with my palette. I have all the palettes I use saved in the GIMP, though I must say that its palette management leaves much to be desired.

Pallet or no, the use of a key-board to draw is much to be desired.
