What is the hardest game you ever played?
This topic arose, because...well...
I WANNA BE THE GUY!!!!
(It's an incredibly hard game. Google it)
Yo Ghost: The Beggining. I could never beg hard enough.
Super Walrus Land: Mouth Words Edition

The immediate things that come to mind for me are Dragon Warrior II, Metroid, and Castlevania II. In fact, I don't think I ever beat Castlevania II. All three of these compounded the difficulty of surviving with the difficulty of figuring out what to do and/or where to go.

Salads made entirely out of calcified stone leaves. Brittle and not very tasty.
... But seriously, IWBTG is pretty much the go-to salad for challenge seekers and lame LPers. It was deliberately designed to be difficult, and was inspired by many of the harder classic salads people have mentioned and will continue to mention in this thread.
I'm going to head in a slightly different direction and nominate Dim and the Jumper series, made in Gam e Maker. I've never fully completed any of them, they're all very tricky platformers where the protagonist is itty-bitty, the levels take up one full screen total (with exceptions, like boss stages), and everything is out to kill you in increasingly infuriating ways.
Actually, Jumper and its sequels are very similar to IWBTG. They even have extra save points added on Easy difficulty, labeled with "EASY" or something. I wonder which came first.

Hardest game is always a odd thing because, it's probably some obscure game everybody including myself has forgot that really gave us the most difficulty. But those games are usually so unpopular and poorly designed it's almost not a factor to figure out.
In that standard, I had beaten like.. I think every game people list on "hardest games ever" lists (Battletoads being an exception).
Anyway I'd probably pick one of these three:
Bart Simpsons VS The Space Mutants (nes)
Ghosts & Goblins (Arcade or NES, they are equally annoying)
Double Dragon III (nes)

I don't think I could fully recall the hardest salad I've ever played, but I do have two gaming accomplishments I'm quite proud of:
1. I finished Battletoads after a month and a half of obsessive gaming (I blame my current less-than-optimal eyesight from having to strain my eyes on the screen so intensely while I made sure I didn't crash on that jet plane level where you have to dodge fireballs and fly through electric gates -- Level 7 for those who want to look it up). The ending was pretty lame in comparison to the salad, but getting to the Dark Queen and defeating her made my summer-long obsession worth it. Consequently, I no longer felt that finishing a salad I started was that important after that moment. That was the crescendo of my gaming symphony.
2. I beat Contra without using the Konami Code. That one took me months, if not years of playing with the Konami Code turned on to master. I got so good at it, and cocky about it, that I decided to try it without the code and I still beat it. It's unlikely I could do that again today. Incidentally, I was never able to finish Super C, which I maintain is a harder salad, unfairly so.
P.S. I really hate this g ame/salad filter. It's gotta be the stupidest forum gimmick in the history of Slim e-Salad or Castle Paradox. Every time I log onto this site now, I cringe. What, am I supposed to laugh every time somebody says, "I think Mega Man is a salad best served with ranch dressing"? For crying out loud! And the saladlist is still broken! Why do I have to manipulate the URL just to look at Page 2???
(*Groan* Even the forum name has a filter. How obnoxious.)

I wanna be the guy is hard, sure, but once you learn how all the tricks go, the game is no longer so. So I don't really count that.
But hardest game I've ever played? Can't say there's a single game that takes that cake, but some of what first comes to mind would be:
Castlevania
Abadox
Zelda II
Though I've almost beat Castlevania, I've beaten Z2 twice, but I've barely got anywhere in Abadox. There are some other games that I know are hard, but I'd have to try to remember them, and today I'm on power-saver mode, so thinking would waste too much energy. :p

I've beaten it with thirty mans. Does that count?
Mega Tact v1.1
Super Penguin Chef

When somebody asks if you are a god, you say YES!

Ghostbusters ftw.
The movie, not the game on the NES.
I forget how far I got in contra honestly, I only have it on the Contra 4 game for the DS (spoiler, and such, for those who don't know: you can unlock contra and super contra in the game's challenge mode.).
I think I got past the waterfall using the 30 lives cheat, but not further. But, sadly, it's not really possible to use my DS as I've "used" it probably far more than nearly any other human. Control pad's busted....
